We require a 24 hour notice to cancel all appointments. All patients need to check with their insurance to see if a referral is required. New patients with TennCare will need to have their first appointment made through their primary care physician. We are currently accepting new patients.

What should I bring to my first appointment? 2018-01-10T19:28:06+00:00

Please make sure that you arrive to our office at least 30 minutes before your scheduled appointment time. This allows us time to input the necessary paperwork and insurance information into our system. To save time, you can also print the New Patient Forms located on our Helpful Forms page and bring them filled out to your appointment or you may fax them in advance.

We also will need your insurance card(s) and photo I.D.
